Working to
deliver sterling community services in Southport and Formby
LANCASHIRE
Care NHS Foundation Trust is now providing community
services to residents in Southport and Formby, since the beginning of May 2017.
The Trust
is providing services in the area after NHS Southport and Formby Clinical
Commissioning Group (CCG) awarded it the contract following a competitive
procurement exercise. The Trust worked closely with Southport and Ormskirk
Hospital NHS Trust and NHS Southport and Formby CCG for 6 months prior to May to
ensure a smooth transition of services for patients and staff.
Sue
Moore, Chief Operating Officer for Lancashire Care NHS Foundation Trust, said:-
"We are very excited to be providing services in
this area and to build on the positive relationships that have been established
with local GPs, the third sector and Southport and Ormskirk Hospital NHS Trust. We
have been closely working with our partners for several months to enable a
smooth transition of services for patients and staff that ensures the delivery
of high quality community services that supports the health and wellbeing of
local people.
We're committed to achieving the vision set in the Shaping Sefton programme and
want to see a better join up of all healthcare with other organisations that are
often involved in providing an individual's care. The aim is to provide services
in a more seamless way and close to people's home whenever it is appropriate and
safe to do so."
Dr
Rob Caudwell, Chair of NHS Southport and Formby CCG, said:-
"It's clear that Lancashire Care understands and shares the
vision we have set out in Shaping Sefton, where all services work better
together and are more able to respond to the differing needs of our patients. We
call this community centred health and care. The views of our residents
contributed to designing this vision and we know, like us, they value the role
of high quality community services play in achieving this.
Whilst Lancashire Care has only recently taken on the management of community
services in Southport and Formby, we have been impressed with the work they've
been carrying out over the past few months to ensure the switch from the
previous provider has been as smooth as possible for both staff and patients. We
are confident that over time Lancashire Care will be able to transform the way
these services operate, resulting in noticeable benefits for our patients and
their carers."
Community
Services is the name for a wide range of care and support commonly provided in:-
clinics, health centres, GP practices and sometimes directly in patients' homes.
These services include:- district nursing, community matrons, blood testing, foot
and leg care, treatment rooms, continence services, pain clinics, diabetes
clinics and speech and language therapies.
